Once again they are blaming “equipment failure” and it is! BUT, the actual failure starts with faulty design that allows less than adequate elements in the position of compromise, albeit, less than pressure rated piping or a remote unvalved pressure gauge. Either they are allowing field personnel to “kluge” things as needed or their piping engineers are “greenhorns”. With the latest rig blow-up in the gulf, it is probably both.
As Avillar said,”... handled the incident properly.” and that is good - doing enough to prevent these things is the “best” and MOST important response.
